Schizophrenia Treated by Homoeopathy: A Case Report

Abstract:

Schizophrenia is one of the expressions of psychosis. A single bad incident can change neurosis to psychosis. Schizophrenia is a major mental illness characterized by persistent defects in the perception or expression of reality, resulting in marked reduction of quality-of-life. A case of Schizophrenia is presented here that was treated successfully by homoeopathic medicines.

Introduction:


Schizophrenia is a heterogeneous syndrome characterized by persistent defects in the perception, thinking, social activity or expression of reality. An acute episode is short and intense, and involves hallucinations, delusions, thought disorders and an altered sense of self. Chronic schizophrenia is a severe, long lasting disability characterized by social withdrawal, lack of motivation, depression, blunted feelings and moderate versions of acute symptoms such as delusions and thought disorders. When one has psychotic symptoms for at least a month but less than 6 months, doctors call it schizophreniform. Many people with this disorder go on to have schizophrenia but for about one-third of people, the symptoms go away. Features of schizophrenia include its typical onset before the age of 45, continuous presence of symptoms for 6 months or more and deterioration from a prior level of social and occupational functioning.

Discussion:

 

The patho physiology of Schizophrenia is the structural brain abnormalities, that means cerebral atrophy (about 5% loss of brain substance), ventricular enlargement, reduced size of specific brain structures and functional brain abnormalities are diffuse cerebral dysfunction, particularly of prefrontal and medial temporal, neuro-chemical brain abnormalities present. The clinical presentations of schizophrenia may be divided into the following 4 domains:

  1. Positive symptoms – Psychotic symptoms, such as hallucinations, which are usually auditory; delusions; disorganized speech and behavior. Positive features are due to Dopaminergic activity in the mesolimbic region.
  2. Negative symptoms – A decrease in emotional range, poverty of speech, loss of interests and drive; the person with schizophrenia has tremendous inertia. Negative features are due to decreased Dopaminergic activity in the meso-cortical system.
  3. Cognitive symptoms – Neuro-cognitive deficits (e.g. deficits in working memory, attention and in executive functions, such as the ability to organize and abstract); patients also find it difficult to understand nuances and subtleties of interpersonal cues and relationships.
  4. Mood symptoms  –  Patients  often  seem  cheerful  or  sad  in  a  way that  is  difficult  to understand; they are often depressed.

Homoeopathic Approach:

Homoeopathy is one of the most popular holistic systems of medicine. The selection of remedy is based upon the theory of individualization, underlying root cause (MIASM) and symptom similarity. The aim of homoeopathy is not only to treat schizophrenia but to address its underlying cause and individual susceptibility. Dr. Hahnemann mentioned about mental diseases in aphorisms 210-230. According to Dr. Hahnemann, the mental diseases are classified under chronic disease as one sided disease, the treatment of which consists of supportive psychotherapy and constitutional medicine. Dr. Hahnemann said that mental diseases are Psoric in origin, chronic in nature and shall be treated with anti-Psoric remedies. According to “§153”, more striking, singular, uncommon and peculiar (characteristic) signs and symptoms of the case of disease are chiefly and most solely to be kept in view in the search of a Homoeopathic remedy. Some  of  the  most  important  homoeopathic  medicines  covering  symptoms  of various types and stages of schizophrenia are: Aconite, Anacardium, Arsenicum album, Aurum met, Aurum mur, Belladonna, Cannabis indica, Causticum, Cimicifuga, Helleborus, Hyoscyamus, Kali brom, Lachesis, Lycopodium, Natrum sulph, Opium, Psorinum, Pulsatilla, Rhus tox, Sepia, Stramonium. The symptoms covering various types and stages of schizophrenia are scattered among numerous mental rubrics in J. T. Kent’s repertory. The direct rubric “Schizophrenia” is found in repertories like Robin Murphy’s repertory under the chapter MIND and in synthesis repertory under the chapter MIND.

Case Report:

Identification:

Name – XXX, Age – 20 years, Sex – Male, Religion – Hinduism, Address – Tangra, Kolkata – 700046, OPD Ticket no – 0411334, Date of first visit – 11/01/2016 of D. N. De. Homoeopathic Medical College and Hospital

Present complaints:

  1. Weakness of memory, cannot remember his lessons
  2. Stiffness in the cervical region; agg. on waking in the morning
  3. Pain in the stomach (sometimes); amel. by eating

Present complaints in detail:

  • Onset: Insidious
  • Duration: Since last 3 yrs
  • Probable Causes – 1) Parental domination especially by his mothe His mother gave him strict instructions regarding everything – what to do, how to dress, what to study and what friends he should have etc. 2) Family History of psychiatric disorder.

Previous treatment: Anti-psychotic drugs since 2 years; the complaints initially subsided but relapsed again.

Past History: Chicken pox at 8 years of age; treated with allopathic drugs.

Family History:

  • Maternal Side – Grandmother died from uterine carcinoma
  • Paternal Side – Father has hypertension
  • Uncle has psychiatric disorder

Personal history: Nothing significant

Homoeopathic generalities:

Physical generals:

  1. Appearance – Dark complexioned, teeth irregular in shape
  2. Appetite – Good
  3. Desire – Milk
  4. Aversion – Nothing such
  5. Intolerance – Nothing such
  6. Thirst – Moderate, 3 liters/day
  7. Tongue – Clean, Moist
  8. Stool – Normal in colour and consistency; semi solid
  9. Urine – Clear
  10. Sweat – Moderate over the entire body
  11. Sleep – Disturbed, anxious and restless at night
  12. Dreams – of fire frequently
  13. Thermal reaction – Ambithermal

Mental Generals:

  1. Weakness of memory, cannot remember what he has read.
  2. Lack of self – confidence.
  3. Foolish behavior, laughs over serious matters.
  4. Always cursing
  5. Various types of delusions; that he was being followed when walking, he would not succeed
  6. Auditory hallucinations; can hear voices calling him by his na

Clinical Examination:

General Survey – (1) Faces – Anxious (2) Build – Stout, healthy (3) Decubitus –

Sitting (4) Pulse – 72/min (5) Nutrition – Good, (6) Blood pressure – 110/80 mm of Hg.

Systemic Examination:  cardiovascular system, alimentary system, respiratory system, urogenital system, and nervous system – No abnormality detected.

Provisional Diagnosis: Schizophrenia (Hebephrenic or disorganized in type).

Pathological investigation reports: Complete blood count, EEG, X-ray of cervical region – antero-posterior and lateral views, thyroid function test were normal.

 

Final Diagnosis: Hebephrenic or disorganized schizophrenia

  4. Delusion that he was being followed when walki
  5. Delusion that he would not succeed in anything; is doing everything wrong.

After analysis and evaluation of symptoms, repertorisation, consultation of Materia Medica and considering the syphilitic predominance of the case, the

medicine selected was Anacardium orientale.

Medicine Prescribed:  Anacardium orientale 0/1, 14 doses, to be taken on alternate days.

Tendency to curse and laugh over serious matters diminished further. Delusions like someone was following him, decreased in frequency. Auditory hallucinations also diminished in frequency.

The patient was remarkably improved. He was jolly and lively. His behavioral abnormalities, delusions and hallucinations entirely disappeared.

Placebo/28 doses to be taken once daily.


Conclusion:

Primitive instincts are suppressed and relegated to the psychic unconscious and organs of the body, thus causing organ symptoms or neurosis which, with or without pathology, contain the patient’s anxiety. From Freud to the present time, most schools of psychoanalysis acknowledge that the constitutional predisposition is an important factor in behavioral and psychic disorders and properly selected homoeopathic remedies can improve the constitution.
